LTC Pharmacist

Job Duties:

Accept and dispense prescription medications

Monitor patient medication compliance and overuse

Ensure compliance with all legal and regulatory medication standards

Verify medication suitability and accuracy for individual patients

Maintain accurate and confidential patient records and pharmacy files

Counsel patients on medication use, reactions, and potential interactions

Provide education on both prescription and non-prescription medications

Deliver patient services such as smoking cessation, blood pressure and cholesterol monitoring

Educate patients on medical device usage (e.g., blood pressure monitors, glucose meters)

Compound medications for internal or external use

Prepare sterile solutions according to proper sterile compounding procedures

Supervise pharmacy staff including technicians and clerks

Maintain appropriate pharmaceutical inventory

Identify and address drug-related issues and communicate with healthcare providers

Collaborate closely with physicians, nurses, interns, and other care team members

Requirements:

Past LTC experience (preferred, but not required)

Active and clear MN pharmacist license
